Stock Certificate Forms
After you create a new stock certificate, you can print the stock certificate and reprint stock certificates that have already been printed. A stock certificate form is any document that has been set up to print information, such as text, titles, or variables, in a specific format to plain paper or pre-printed stock. Even though you use the same report to print the list or forms, you do not need to print a list in order to print forms, or vice versa.
Do this...
1. Open Connect Utility Management > Locations > Stock Certificate List/Forms.
2. Select a report title.
Example: Stock Certificate List/Forms - Stock Certificate [Caselle Master]
3. Click to select Print Stock Certificate Form.
4. Do you want to create the proxy voting history?
What is it? The proxy voting history is a record of the votes that have been submitted by the shareholder for voting ballots from previous elections.
When should I create proxy voting history? It's time to create proxy voting history when you send the proxy voting forms. Creating the proxy voting history prepares the system to record the proxy votes that you will be receiving from the property owners.

Yes, include the proxy voting history. Click to select Create Proxy Voting History.

No, do not include the proxy voting history. Click to clear Create Proxy Voting History.
5. Skip the Mailing Date.
The system only records the Mailing Date when proxy voting records are created.
6. Skip the Due Date field.
The due date is assigned to voting ballots. You can use the current date as the due date since the field does not apply to printing stock certificates.
7. Do you need to change the form type?
What is it? The form type tells the application how and where to print text and variables.

No, use the default form type. Continue to the next step.

Yes, change the form type. Use the options on the Forms tab to select a different form type.
8. Do you want to print a test form to plain paper?

Yes, print a test form to plain paper. Load plain paper in the printer, then click Print (CTRL+P). Place the printed form on plain paper over the pre-printed paper stock, now hold both sheets in front of a bright light.
You're good to go if the printed information appears in the correct location on the pre-printed paper.
On the other hand, if there is an alignment issue you may need to tinker with the margins on the Form Print Settings tab or the printer settings. -
No, print forms now. Load the printer with the paper stock that you'll be using. Click Print (CTRL+P).
9. Review the printed form for correctness.
You're done.
