Printing Forms 1094-C and 1095-C

1. Open Connect Payroll > Government Reporting > 1094/1095s.

2. Use the Title menu to select the report titled 1094/1095s - 1095-C and 1094-C [Caselle Master].

3. Use the Tax Year menu to select the reporting year.

4. Set up the checkbox titled Mark This Form as the Authoritative Transmittal.

"In the case of an Aggregated ALE Group, each separate employer (referred to in these instructions as an ALE Member or employer) must file its own Authoritative Transmittal. Although an employer may file multiple Forms 1094-C, one “Authoritative Transmittal” Form 1094-C, identified on line 19, Part II as the Authoritative Transmittal, must be filed for each employer reporting aggregate employer-level data for all full-time employees of the employer" (2015 Instructions for Forms 1094-C and 1095-C, p. 2).

5. Click to select the Form tab.

6. Check the form definitions that have been selected for 1094 Form and 1095 Form.

The field titled 1094 Form should be 1094-C [Caselle Master] and the field titled 1095 Form should be 1095-C [Caselle Master].

7. Load the paper stock for Form 1095-C in the printer.

8. Click Print (CTRL+P).

Form 1095-C prints for employees.

9. Load the paper stock for Form 1094-C.

If the information prints on Form 1095-C, you can re-print the last page of the report. The last page is the information that should print on Form 1094-C.

Form 1094-C prints for the employer.
